Nicolau's Syndrome is a rare necrotic, livedoid dermatitis that can occur after intramuscular injections of various drugs. It can cause ischemic necrosis of skin and underlying adipose tissue. The actual cause of NS is not clear though inflammation, vasospasm and thrombo-embolic occlusion of blood vessels have been proposed. In our case report, we report the case of a 55 yr old woman who developed NS after an intramuscular injection in her right gluteal region 3 weeks back and then complications started as swelling, ulcer, necrosis at injection site. This case shows that proper care should be taken during the administration of any parentral injections by health care personnel to prevent further complications of an already existing illness.
